Admission Requirements
ADMISSION REQUIREMENTS
To apply for admission to the school an application must be submitted with a copy of a child’s birth certificate. A registration fee is also required. Forms required by school, i.e. medical release, form 3231 (immunization record) and hearing, vision and dental screening results – should be on file within 30 days of enrollment. It is the responsibility of the parents or guardians to maintain accurate information with the school and to report any changes in information to the schoo1.
In order for a child to be enrolled in the school, each parent or guardian will provide to the school:
- A completed application (both sides/all blanks filled in)
- A copy of the child’s birth certificate
- A copy of state immunization form 3231
- A signed emergency medical release form
- A signed statement of receipt of school handbook and understanding of its content
- A vehicle medical form for children ages 3 years and up
A copy of hearing, vision and dental screenings for children in the Pre-K, Kindergarten and 1st
