A current immunization record signed by physician or from a Health Department is required by state regulations. If a child’s record of up-to-date immunizations (signed by your physician) is not complete at time of enrollment or within thirty days after enrollment, the child will be excluded from the program.

According to Georgia State Law, children who are ill cannot remain at school. Parents will be notified immediately of a fever of 100 degrees or more and any illness or serious injury. A child must be free of fever 24 hours before returning to school.

We will inform you if your child has been exposed to a communicable disease. Children shall not return to school after having a communicable disease unless they have been released by a physician and no symptoms remain. Parents will be informed of injuries that occur at school. You will be notified if your child is exposed to any harmful substance in the school environment, such as lead.

No medication will be administered by the staff except by written authorization of the parent. All prescription medicine must include prescription number and be in the original container and labeled with the child’s name. A medical form must be filled out each day your child is on medication. Forms will be provided by the school. Over-the-counter medicine must have a doctor’s note in order to be administered.

If you plan to be away from work, please leave the phone number where you may be reached in an emergency. We need to be able to contact you at all times. If we cannot reach you in case of accident or illness in an emergency situation, we will take the child to the nearest medical emergency center to be treated. You will be asked to sign a form giving this permission. School accident insurance covers what your own policy does not pay.
